Audio Book Club - declined to received monthly offer - despite decline, offer received in mail

Every month I would return club card saying do not send this months offer. Than about three days later the club offer would show up in the mail. I would write refused and send it back.

Than I received a bill for 4 or 5 purchases that all had been returned. I wrote a letter stating that they had all been returned and had not been agreed to be accepted.

The story goes on and on after three letters and a phone call they are still trying to bill me for one purchase I do not have and are tacking on late fees to boot.
