Missing debit card, check books

TWIMC: Dear Sir, I had sent two letters to the Bangalore branch on the 14th (May 2001):

1. registering a cahnge of address and requesting a replacement debit card

to be sent

2. adding Ms. Konina Sircar to the account, and requesting a debit card and

checkbooks to be sent to the same address. I have not yet received anything from the bank. Please redress this as

soon as possible. I enclose the address again below. Thanking you, Lata Sircar,
