Second notice of an unauthorized issue of a Care Credit card in my name. On may 9, 2004 I notified CARE CREDIT /GE CAPITAL CON CC that you had issued a credit card under my name without my authorization. It should have been issued solely to Sharon B. Albert’s name only. I have no responsibility and I do not take responsibility for Ms Albert’s credit card debts. My name continues to be used without my authorization on the card, and I continue to receive phone calls. At that time, GE CCCC also without my authorization, GE CCCC accessed my personal checking account and debited $300. You kindly acknowledged the error, and refunded me. Thank you. Now, I again request that you terminate the card issued in my name without prejudice to me. And further I request that Care Credit and/or your representatives cease calling me at my home. To: GE CCCC From: Yvon Subject: Unauthorized transaction by CARE CREDIT /GE CAPITAL CON CC On may 8, 2004, I received a notice from your firm informing me that I had authorized a transaction from my University Credit Credit Union in the amount of $300. I did not authorize such a transaction, and I request you refund to me the amount of $300. withdrawn illegally from my personal bank account. In addition, CARE CREDIT /GE CAPITAL CON CC issued a credit card under my name, again without my authorization. It should have been issued to Sharon B. Albert’s name only. I have no responsibility for Ms Albert’s credit card debts. Please attend to my request in the briefest of delays. Thank you.
