Don't go to this doctor's office for anything! I went because I wanted to lose weight and they treated me for back pain. When I was filling out paperwork before my first visit, I signed a form about "physical therapy". Since I was in for weight loss, I didn't think anything of it. Talk about "bait and switch". They want everyone to sign this form because they know they will have the patient come back for something other than their original problem. I should have been more careful and conservative with this doctor, but what they have done is unethical! Now they are charging me $100 extra dollars that I shouldn't have owed them because I didn't mean to go in for this "physical therapy", that just what they scheduled the appointments for!
