Box 920041 Dallas, TX 75392-0041 RE: Erroneous billing of non-basic service charges To whom it may concern: This letter shall serve to document my attempts to rectify your continued erroneous billing. In addition, this shall outline a course of action to remedy the situation. I have been charged $32.65 monthly (inclusive with my Verizon bill) for a Yellow Pages Directory listing that I have requested removed since September 2004. I have repeatedly asked that the listing be discontinued to both OAN and Verizon, and spent hours on hold with OAN service numbers (800) 944-9646 and (800) 892-8424. Further, on the one instance that I was able to contact an OAN representative, the service was not discontinued and the charge remained in full force on my next bill. My last attempt to contact OAN on November 23, 2005 culminated in a conference call. The Verizon representative, Rachel, and I were placed on hold for 72 minutes until she informed me that she had to go home. At that point, she terminated the call. Comes now, I have spent a lot of time, and paid a total of $489.75 to AON for a listing of my practice that has not existed since September 2004. Effective immediately, cease and desist my Yellow Page Directory listing and billing included in my Verizon statement. Excluding my inconvenience, I also request that you provide compensation in the amount of $489.75 to me at the address listed above, via mail, no later than January 15, 2006.
