1-800-496-1132 My husband was in the hospital at Denton Community Hospital on 2-9-04. Chest x-rays taken, 2 views for a total charge of $50.00. I have Blue Cross/Blue Shield Health Select insurance who took care of payment on 3-12-04 in the amount of $19.51. Services received were coordinated by your selected network provider. The amount of $30.49 benefits were paid at the higher in-network payment level. The total covered benefits approved for the claim: $19.51 to Hostetter MDPA, Susan N. on 3-12-04. Amount we owe provider (Denton Radiology) $0.00. A letter was sent to me from Blue Cross/Blue Shield of Texas dated September 9, 2004, stating the claim was processed on March 12, 2004. A payment in the amount of $19.51 was issued to Susan N. Hostetter MD PA on March 12, 2004, check number 15882423. The patient responsibility is for the amount of $0.00. I have contacted Denton Radiology on 3-12-04, 4-25-04, 7-13-04, 10-04, 1-05, 4-30-05, 5-5-05. I have been told by Denton Radiology that the check came into the office and had been posted to another account instead of my husband account and that they would get it cleared up and I would not receive any more notices. But again this week I have received a FINAL NOTICE if they do not received the full amount, your account will be turned over to a collection agency for further action. I have made several attempts to contact the Denton Radiology and they have admitted it is their mistake, but they continue to harass me by sending out collection notices. It is very obvious that Denton Radiology has problems within their system and not able to handle patients accounts. When I contacted Denton Radiology today, I wanted to speak to the head supervisor today and was put off again. I have been patience for over a year, since 2-9-04 and now I want this matter cleared up. Maybe you will be able to assist me in this matter. Please send response to address below.
