HHLD bank - Could you tell me who to contact to cancel this "supposed" credit card I have with HHLD bank. I just got turned down for a loan because this was out there and I never applied for a credit card with this bank.

Could you tell me who to contact to cancel this "supposed" credit card I have with HHLD bank. I just got turned down for a loan because this was out there and I never applied for a credit card with this bank. Plus, it's showing a limit of $9,500 when I only have cards with a $4000 limit.
