Sony CD500 Mavica digital camera - still have not received the rebate

I purchased a Sony CD500 Mavica digital camera December 23, 2003, mailed in all required info for rebate of $50.00 two weeks later and as of May 28, 2004 I still have not received the rebate.

Any suggestions or phone numbers to call would be appreciated.
