Inphonic - Rebate on T-Mobil phone purchase

I purchased a T-mobil phone 4-16-04 Order #348332558 through Inphonic on line. They offered a $50 rebate from T-Mobil and $100.00 rebate from Inphonic. I had to wait 2 months to send them copies of my T-Mobil bill to get the rebate. Then I realized they did not enclose the form with my phone. I have called 5 times and held on the line 45 minutes each time.

I have e-mailed for the rebate form 4 times and received generic answers. I spoke to them again today and got the same speel - I believe this company has no intention of giving me my rebate form and paying me the $100.00 they owe me. When I call the rebate department or e-mail them they say they only handle payment and can not send me a form.
