Dish Network - Dish Tv headache

We decided to switch from cable to Dish Network.Their workcrew showed up to our place two hours past the time they were scheduled.Instead of installing the cable down the wall. they ran the cable down the outside of the condo and in through the kitchen windows leaving the wires dangling everywhere in the kitchen.When they connected to the tv they somehow screwed up the picture where it was reduced from a full screen down to a small box which has never been fixed.We got tired of being overcharged by Dish after six months ( We were never billed the $20 a month like we were promised.We called and cancelled our service.We were told they would send out boxes for the equipment.They never arrived.After a month someone from Dish arrived at our door to pick up the equipment.Two months after that Dish took $300 from our checking account saying we never returned their equipment.They told us they would return the money when they found the equipment.That was a year and a half ago and we still have not received any money back.
