Dish Network - Refuse to allow me to speak to a manager

I've just recieved a email that my bill has risen to $358.98 from $25.99. 1st person I spoke to said its for the reciever they sent me but NEVER installed. Its still sitting waiting for they to come hook it up.. I said I will send it back tomorrow. NO... They can not put a hold on the charge til they recive it back. May I speak to a MANAGER?? Please hold? Then disconnnect.. They have done this 4 times in a row... My best option now is to close my checking account to stop the possible auto withdrawal...
