Honestly, too much to list but, here are some highlights.

They trust 9 year old computers in cars,

They don't test your car if there's a dash light on. In other words, even if the car isn't polluting, it still fails just because the computer is malfunctioning. (this is what happened to me). The testers are unwilling to communicate what the real issues are with the failed test and your car can also be randomly selected for govt. testing so make sure you say NO to that.
