I need a disability paper filled out and signed by this dr.s office so my disability income will continue while I am off work. Judie Kerby- the person who does this refused to do so unless I pay $50 on the bill. They have gotten 80% of their money from the insurance I pay out of my own pocket at $650 per month. Medicare is supposed to kick in to pay a large chunk of it but they are slow. My wife is disabled with a heart condition and is unable to work full time and we have very expensive copays for prescriptions
