Iam receiving the bills after diconnection of the tata number

I vikas agarwal from hyderabad has diconnected the phone number 9247366447 last month but still I am receiveing the bill when I spoken to the customer care regarding this there are given the fax number to fax the letter but from last five day the fax number is also not working. I have attached the post installation refund form please find it.
