mistakes in passport

Hi,

I had submitted the application for a new passport of my wife at Passport office, Ghaziabad. After paying lots of money for the verification and waiting for a long time of about four months, when I got the passport then there were lots of mistakes in it. Neither the mother's name is correct nor the single line of the address is correct. It looks like they have totally shuffled the each and every letter of the the address.

I am currently in Bangalore, and not able to decide what to do with that. Ho to get it modified.....
