Today I received a bill on my credit card for 2/04/07 $ 9.95 and 2/04/07 $9.95, along with a credit 2/04/07 of $ 9.95.

Please refer to your records and you will see that I canceled both these services on 12/14/06.

Therefore, please issue a credit for these incorrect charges as soon as possible. My account is Chase xxxx xxxx xxxx 5604.
