Asurion cell phone insurance The worst customer service ever! They were going to mail me an affidavit that I had to fill out stating why my cell phone was not working and why I wanted it replaced. Then I was supposed to have the affidavit notarized by a notary public and send it back to them along with a copy of my bill (I'm on a corporate plan where the bill goes direct to corporate accounting) and a copy of my drivers license. Then after they received these documents in the mail, someone from Asurion would evaluate it to determine if they would allow me to get an insurance replacement of my phone (plus a $50 deductible).
