Health care provider. 7124 Miami ave. Cincinnati ohio.

We had an appointment for late evening Monday, prior to that

on Saturday the weekend before at about late after noon

i received a notice in the mail that my insurance was

canceled, I called the insurance company to confirm this

it was true, at midnight 2days before my appointment,

on a Saturday, i was loosing my coverage, due to a layoff,

which followed in the letter. I then called, left a message explaining things,

I also called Monday morning. the provider is still trying to charge me for a missed

appointment. I called them personally, talked to them, I even have

certified letters, and the insurance company, to confirm

everything!!! they still want me to pay for a missed office visit..
