disconnection of sky account

Dear Sir /Madame

I wish to get account no 410000539628 disconnected permanently.I have

tried this over the phone on numerous occasions but because there seems

to be some mix up in details associated with verification of account

owner have been unsuccessful.I have recieved one bill in the meantime

and expect to see another one soon.I have not been living at the

address or using the card in the last few months .The address is 18 Ash

Haven Oranmore Galway.I would appreciate your help here.
