On April 19, 2006, I had a dental appointment with Round Rock Dental Associates for 11:00 am, located at in Round Rock, Texas. The appointment was scheduled in 2 to 3 weeks prior to the appointment time. Round Rock Dental Associates require a 24 hour notice for any cancellations. The night before my appointment, I learned that my spouse could not watch our children during my appointment due to some work-related deadlines. The next morning I called a friend who had baby-sat my daughter before, to ask if she could watch my children while I went to the appointment. However, there was no answer when I telephoned her so I called my spouse and told him that I would have to take my children along with me to the appointment.

When I got to Round Rock Dental Associates, I arrived 20 minutes early to fill out new patient paperwork. Shortly after completing my paperwork, the staff called me to begin the exam. I had my two daughters in a double stroller and brought them back with me to the exam room. My girls were very well-behaved for most of the appointment and then my youngest daughter who is 6 months old got sleepy.
